Electronic Medical Record (EMR)

Electronic Medical Record (EMR), developed in collaboration with the Aga Khan Health Service, Pakistan, is a web-based EMR platform. The platform includes separate interfaces for each type of healthcare provider (unit clerk, nurse, and physician) interacting with the application for efficient patient management and record-keeping.

EMR is currently operational at the Aga Khan University Family Medicine Center (FMC) in Karachi, Pakistan.

Modules

Unit Clerk

The module includes patient registration; patient search using filters and appointment scheduling.

Nurse

The module includes visit history, an initial assessment form for physical examination, and a color-coded patient card indicating their condition (stable or unstable) based on the assessment.

Physician

The module includes detailed patient history, current consultation form, data approvals by supervisors, and appointment management.

Admin Panel

Enables the administrators to set up and maintain levels of information for patients, doctors, and nurses.

Summarizes patient’s chronic and acute medical history, along with their current medications.

Current Appointment

Provides a comprehensive overview of the patient's medical history and diagnoses, including medical surgical history, chief complaint, allergies, and family history.

Integrated Medical Calculators

integrates several indicators and calculators such as those for recording the patient's mental health (PHQ2 & 9), ASCVD scores, Framingham score, creatinine clearance, and CHA2DS2-VASc Score for Atrial Fibrillation Stroke, among others.

Examinations & Treatment Plan

Includes laboratory and radiology test results, and a versatile treatment plan that includes options like prescriptions, health education, follow-ups, and referrals.
